Amoxicillin/clavulanic acid+aminoglycoside as empirical antibiotic treatment in severe community-acquired infections with diagnostic uncertainty

Abstract

Diagnostic uncertainty is common in the emergency room and multidrug-resistant bacteria emerge in the community setting, implying to establish the most efficient empirical antibiotic therapy (eEAT). Our aim was to identify such eEAT, considering that in case of DU with severe clinical presentation, most prescribers would propose an empiric combination (EC). The medical dashboard of our ward records prospectively 28 characteristics of each hospitalization including hospitalization motive, final diagnosis, and all antibiotics prescribed. All patients with community-acquired bacteremia (CAB) were included. DU was defined by a discrepancy between suspected diagnosis in the emergency room and final diagnosis. eEAT was defined by in vitro activity of at least one prescribed compound. Finally, independently from the dashboard, we retrospectively compared 2 CTs: amoxicillin/clavulanic acid (AMC)+gentamicin (G) and cefotaxime (3GC)+G. One thousand thirty-four patients with a final diagnosis of CAB were identified from July 2005 to June 2018, including 357 DU (35%) at baseline. eEAT (n = 553) was associated with a trend towards a lower death rate compared to inefficient therapies: 5.4 vs 10.0% (p = 0.053), and effective antibiotic reassessment was the most protective factor against an unfavorable outcome: 0.34 (0.16–0.71). Bacteria involved in case of UD were resistant to AMC+G and to 3GC+G in 8.1% and 12.8% of patients, respectively. Diagnostic uncertainty was a frequent event requiring antibiotic reassessment. As the latter was not systematically realized, the best eEAT is required and AMC+aminoglycoside should be considered.

Characterization and in vitro and in vivo assessment of poly(butylene adipate-co-terephthalate)/nano-hydroxyapatite composites as scaffolds for bone tissue engineering

Abstract

The polyester for bone tissue engineering is produced using different concentrations of hydroxyapatite. Poly(butylene adipate-co-terephthalate) (PBAT) solutions containing different concentrations of nano-hydroxyapatite (nHAp) (1, 2, 3, 4, 5 and 6% wt) were evaluated to assess their potential to produce scaffolds via electrospinning. The characteristics of these solutions were evaluated using surface tension analysis. Different solutions were electrospun and characterized using scanning electron microscopy, X-ray diffractometry (XRD), infrared spectroscopy (FTIR), and differential scanning calorimetry analysis. In vitro and in vivo experiments were performed using MG-63 osteoblast cells and male Wistar rats. The MTT assay was used to evaluate the cytotoxicity of scaffolds. Microtomography and a three-point bend test were used to analyze parameters of bone neoformation 4 weeks after implantation. We observed a requirement for solutions containing nHAp to reach chemical stability to produce ultrathin fibers. FTIR and XRD data demonstrated the presence of carbonate and phosphate groups, and thermal analysis showed a reduction in crystallinity of the nanocomposites when the concentration of nHAp was increased. None of the scaffolds analyzed demonstrated cytotoxicity when compared to controls. All of the PBAT/nHAp scaffolds analyzed promoted bone repair; however, a solution of PBAT with 3% nHAp improved bone volume, force, and stiffness when compared to controls.

PD-L1- und PARP-Inhibition - neue Therapieoptionen beim metastasierten triple-negativen Mammakarzinom

Patientinnen mit triple-negativem Mammakarzinom und PD-L1-Überexpression auf Tumor- bzw. Immunzellen profitieren von der Kombination nab-Paclitaxel plus Atezolizumab, berichtet Prof. Michael Untch. Die übrigen sprechen meist gut auf die Standardchemotherapie mit nab-Paclitaxel plus Platin an. Für Patientinnen mit BRCA1- oder 2-Mutation stehen bald PARP-Inhibitoren zur Verfügung.

Literaturtipp: Neue Aspekte in der Therapie von Schilddrüsenkarzinomen 

Anfang Juni 2018 fand in Chicago der jährliche Kongress der American Society of Clinical Oncology (ASCO) statt. Präsentiert wurden v. a. Studien zur Therapie des radiojodrefraktären differenzierten Schilddrüsenkarzinoms, des anaplastischen Schilddrüsenkarzinoms sowie zur Immuntherapie. In ihrem Review stellen die Autoren klinisch und präklinisch relevante Studien vor und interpretieren die Daten.

Neuer Wirkstoff bei MDS, schonendere Sequenz bei AML

Prof. Dietger Niederwieser berichtet über Neuigkeiten in der Therapie von MDS, AML und ALL: Für MDS-Patienten gibt jetzt einen neuen Wirkstoff, der die Anämie und damit die Transfusionsbedürftigkeit verringert. Bei älteren, oft komorbiden AML-Patienten hat sich eine schonendere Sequenztherapie als vorteilhaft herausgestellt. Auch die ALL-Therapie mit CAR-T-Zellen war wieder ein wichtiges Thema, so Niederwieser.

Artemisinin and its derivatives: a potential therapeutic approach for oral lichen planus

Abstract

Background

Oral lichen planus (OLP) is a common T-cell-mediated oral mucosal disease, whose pathogenesis mainly includes antigen-specific and non-specific mechanisms. As a refractory chronic inflammatory disease, there is still no curable management for OLP till now.

Findings

Artemisinins are a family of compounds that are widely used as frontline treatment for malaria worldwide. In addition to its well-established antimalarial properties, emerging evidence hints that artemisinin family drugs also possess preferential immunoregulatory and anti-inflammation properties, such as modifying T lymphocytes' activation and cytokines release, modulating Th1/Th2 balance, activating regulatory T cells (Tregs), modulating inflammatory signaling pathways, as well as acting on non-specific mechanisms of OLP. However, there is still no report focused on the influence of artemisinins on OLP.

Conclusion

This review outlined the data-based immunomodulatory effects of artemisinins on different immune cells in conjunction with their therapeutic prospective with regard to the pathogenesis of OLP, suggesting that artemisinin and its derivatives might be possible candidates for treatment of OLP.

Radiation exposure in infants with oesophageal atresia and tracheo-oesophageal fistula

Abstract

Purpose

Oesophageal atresia and tracheo-oesophageal atresia require surgical repair in early infancy. These children have significant disease-related morbidity requiring frequent radiological examinations resulting in an increased malignancy risk.

Methods

A single-centre, retrospective review was performed of radiation exposure in children with OA/TOF born 2011–2015. Medical records were reviewed to determine the number and type of imaging studies involving ionising radiation exposure enabling the calculation of the estimated effective dose per child over the first year of life.

Results

Forty-nine children were included. Each child underwent a median of 19 (IQR 11.5–35) imaging studies, which were primarily plain radiography (median = 14, IQR 7–26.5). The overall median estimated effective dose per patient was 4.7 (IQR 3.0–9.4) mSv, with the majority of radiation exposure resulting from fluoroscopic imaging (median 3.3 mSv, IQR 2.2–6.0). 'Routine' postoperative oesophagrams showed no leak in 35/36 (97%) with the remaining study showing an insignificant leak that did not alter management.

Conclusions

Careful consideration should be given to the use of imaging in OA/TOF to minimise morbidity in these vulnerable infants. Oesophagrams in children without the symptoms of anastomotic leak or stricture should be discontinued. Standardisation of monitoring protocols with regard to radiation exposure should be considered.
